This Claim Form should be filled out online or submitted by mail if you received a Notice of Data Incident from GATE Petroleum Company (“GATE”) on or around November 9, 2022, and you would like one or more of the following:

  1. two (2) years of free credit monitoring services; and/or

  2. up to $500 in reimbursement for (a) unreimbursed costs and expenditures incurred in responding to notice of the Data Incident and/or (b) compensation for time spent remedying issues related to the Data Incident up to 3 hours at $25 per hour; and/or

  3. up to $4,000 in reimbursement for extraordinary unreimbursed costs or expenditures that are fairly traceable to the Data Incident.

You may get a check or electronic payment if you fill out a Claim Form, if the Settlement is approved by the Court, and if you are found to be eligible for a payment.

This website describes your legal rights and options. To obtain the Settlement Notice and find more information regarding your legal rights and options, please visit the FAQs or call toll-free 866-675-3062. You must submit a claim online by June 27, 2025.

You are only eligible to file a claim if GATE sent you notice that your personal information may have been potentially accessed by an unauthorized actor as a result of the Data Incident occurring on or around September 20, 2022 to September 22, 2022.

By submitting a claim and signing the certification you are verifying that you were notified of the Data Incident announced by GATE on or around November 9, 2022.

To complete this online claim form, you must login. You can find your Unique ID & PIN printed on the Notice that was sent to you. If you lost or do not know your Unique ID provided in the Notice, you may contact the Claims Administrator at info@GatePetroleumDataIncident.com.

If you wish to submit a claim for a settlement payment via standard mail instead, please print and complete the Claim Form and send it to GATE Petroleum Settlement, PO Box 25226, Santa Ana, CA 92799, postmarked by June 27, 2025.
